Form Responses

View, attach, detach, and follow up on form submissions

This guide covers viewing, filtering, attaching, and managing form submissions from your clients.


Responses Tab on the Form Detail Page

Every form has a dedicated Responses tab showing all submissions for that form.

Accessing Responses

  1. Go to Fields & Forms > Forms tab

  2. Click on the form you want to review

  3. Select the Responses tab

What You See

Each response row shows:

  • Client name (pet owner)

  • Submission status (Pending or Completed)

  • Submitted (the send date for pending rows, or the completion date for completed rows)

  • Sent via - SMS, email, both, Self-service booking (the form was collected in online checkout), or Not sent, plus the mobile or email used when Petboost sent it

  • Date completed (when the client submitted their answers)

From a submission you can Change or Remove the appointment link. Remove only clears the appointment. It does not delete the submission or reset once-per-client.


Status Filtering

Use the status filter to focus on what matters:

  • Pending - the form has been sent (or created for the client) and is not yet completed. Status Pending is the sent-but-not-completed state. The Sent via column shows whether Petboost sent the form by SMS, email, or both, or that it was collected during a self-service booking, and the mobile or email it used. Use this to follow up with clients who have not completed their paperwork.

  • Completed - the client or a staff member has submitted answers. Use this to review recent submissions or verify answers.

Click the status filter buttons at the top of the Responses tab to toggle between views.


Viewing Individual Submissions

  1. Click a completed submission row in the Responses tab

  2. The full submission opens showing:

    • All questions and the client's answers

    • Signature images (if a Signature field was included)

    • Submission date and time

    • The form version that was used (if the form has been edited since submission)

Submissions are read-only. You cannot edit a client's answers after submission. You cannot delete a submission.


Submissions on Customer Profiles

Form submissions live on the customer profile Fields & Forms tab, not the Paperwork tab. Paperwork is agreements and uploaded documents.

Fields & Forms Tab

  1. Navigate to a pet owner's profile

  2. Open the Fields & Forms tab

  3. Form Submissions lists pending and completed forms for that client

  4. From here you can Send Form, Resend a pending form, Attach a submission to an appointment, Fill on Behalf, or Fill a Pending row

Forms on the Appointment

The appointment Forms & Responses section shows submissions linked to that booking. From there you can Send, Resend, Attach, or Detach.

If the booking has several completed forms, they collapse into an accordion. Open the form you need.

Detach only clears the appointment link. Status stays the same. No new send is created. Once-per-client stays satisfied if the form was already completed.

Attach Rules

  • Attach only to an appointment for the same client

  • You can move a submission from one of that client's appointments to another

  • You cannot attach a submission to a different owner's booking

  • If a client completes a still-unlinked form and then books within 24 hours, Petboost attaches that completed submission to the new booking instead of sending again

  • Auto-link on completion only happens when exactly one matching upcoming booking exists. If more than one matches, the submission stays unlinked until you Attach it


Completion Tracking

Per-Form Tracking

On the form detail page, you can see overall completion metrics:

  • Total forms sent

  • Total completed

  • Total pending

Per-Client Tracking

On the customer profile Fields & Forms tab, you can see which forms have been completed and which are outstanding for that owner.

Following Up on Pending Forms

For clients with pending forms:

  1. Filter the Responses tab by Pending status

  2. Check the send line to see whether the form went by SMS, email, or was not sent

  3. Resend the same pending submission from their Fields & Forms tab or from the appointment

  4. Or contact them directly to remind them

Resend re-notifies the same pending. Send Form creates a new pending (blocked if the form is once-per-client and already completed).


Fill a form on behalf

Pending means the form has been sent and the client has not completed it. Staff can complete that row, or start a new submission, from the customer's Fields & Forms tab.

  1. Open the customer's profile

  2. Open the Fields & Forms tab

  3. Click Fill on Behalf for a new submission, or click Fill on a Pending row

  4. In Fill form on behalf, select the Form if you started a new submission

  5. Optional: choose For pet (optional), or leave No specific pet

  6. Optional: turn on Notify client

  7. Enter the answers, then click Complete Form

The submission is attributed to you. Completing it counts as Completed for Once per client. See Intake Forms Guide.


Frequently Asked Questions

Q: Can I export form responses?

A: Submitted values that are linked to custom fields automatically populate on profiles. For bulk review, use the Responses tab which shows all submissions in a list with filtering.

Q: What happens to responses if I edit the form?

A: Existing submissions keep their original answers based on the form version at the time of submission. New sends use the updated form structure.

Q: Can a client resubmit a form?

A: If frequency is Every appointment, a new submission is created for each booking. If frequency is Once per client, any completed submission for that owner account blocks another send. Detach does not allow a resubmit.

Q: Can I delete a submission?

A: No. Individual submissions cannot be deleted. They are retained as a permanent record on the customer profile.

Q: How long are submissions stored?

A: Submissions are stored permanently and remain accessible on the customer profile Fields & Forms tab and the form Responses tab for as long as your Petboost account is active.

Q: I removed the appointment link. Will the form send again?

A: No. Remove / Detach only clears which booking the submission sits on. Completed stays completed. For Once per client, change frequency or Duplicate the form if you need another send.
