Sending Forms to Clients

When to collect, send, resend, once per client, and silent booking

This guide covers how intake forms reach clients: When to collect, automatic sending on booking, manual send and resend from profiles, staff fill-on-behalf, and shareable links.


When to Collect

There is no separate "Auto-send after booking" toggle. Open the form, go to Settings, and set When to collect:

  • Send after booking - the form goes out by SMS and email after a booking is created for a linked service

  • Required before booking - pet owners must complete the form in self-serve checkout before they can confirm. If staff create the booking, the form is sent afterwards instead

Changing When to collect turns auto-send on. The form must also be Active, not archived, and linked to the service (or set to all services).


Auto-Send on Booking

When When to collect is set and the form is active, a pending submission is created for bookings of linked services (unless frequency or a staff choice skips it).

How to Configure

  1. Go to Fields & Forms > Forms tab

  2. Open the form

  3. In Settings, set When to collect to Send after booking or Required before booking

  4. Link the form to specific services, or all services

  5. Set frequency: Every appointment or Once per client

  6. Save the form and keep it Active

What Happens When a Booking Is Created

  1. Staff or the pet owner creates a booking for a linked service

  2. The system checks frequency (see below)

  3. If a send is required, the pet owner receives both:

    • An SMS with a direct link to the form

    • An email with the form link and instructions

  4. The submission appears as Pending in the form's Responses tab. The Sent via column shows whether Petboost sent the form by SMS, email, or both, or that it was collected during a self-service booking, and the mobile or email it used.

Required before booking is different for self-serve: the form appears in checkout and must be completed before confirm. Staff-created bookings still send afterwards.

Frequency

  • Every appointment - a new pending submission on each booking for a linked service. An old pending does not block a new send.

  • Once per client - per pet owner account, not per pet and not per service. Any completed submission (including staff fill-on-behalf) blocks auto-send and manual Send Form. Completing the form for one dog blocks it for a sibling on the same owner account.

Unlinking a completed form from an appointment does not reset once-per-client. Submissions cannot be deleted. To collect again, change frequency to Every appointment, or Duplicate the form and send the copy.

Per-Service Configuration

Different forms can be linked to different services:

  • A boarding health questionnaire linked only to boarding

  • A grooming waiver linked only to grooming

  • A general liability agreement linked to all services with Once per client

Configure service links in the form Settings. Only bookings for linked services trigger auto-send. One form on a booking with several matching services is sent once. Several forms on one service each follow their own frequency.

Courses can be linked the same way, using the lesson service.

Silent Booking

If staff choose Book silently (skip both SMS and email on the booking), form auto-send is skipped too. No pending submission is created by the booking.


Manual Send and Resend

Send and resend live on the customer profile Fields & Forms tab, not the Paperwork tab. Paperwork is agreements and uploaded documents.

You can also send and resend from the appointment Forms & Responses section.

Send Form

  1. Open the pet owner's profile

  2. Open the Fields & Forms tab

  3. In Form Submissions, click Send Form

  4. Select the form

  5. Confirm delivery (SMS and email)

Send Form creates a new pending submission. If the form is Once per client and that owner has already completed it, send is blocked with "This client has already completed this form."

Resend

Resend re-notifies the same pending submission. It does not create a second pending.

Completed, archived, or inactive forms cannot be resent. Use Send Form only when you need a new pending (and frequency allows it).


Staff Booking: Fill for Owner or Send to Owner

When staff create a booking for a service with a linked form, they choose:

  • Fill for owner - staff complete the form during the booking. The client does not get a form SMS or email for that send.

  • Send to owner - the form is sent to the client after the booking is created.

Either choice suppresses the background auto-send so you do not get a duplicate pending.

Use Fill for owner for phone bookings and walk-ins when the client is present.


Shareable Links

Every active form has a shareable link you can copy and distribute through any channel.

How to Copy the Link

  1. Go to Fields & Forms > Forms tab

  2. Open the form

  3. Click the Copy Link button

Where to Use Shareable Links

  • WhatsApp messages to individual clients

  • Facebook Messenger conversations

  • Your website (embedded as a button or link)

  • Social media bios

  • Printed materials with a QR code

The link opens the form in a browser. The client fills it in and submits. The submission is recorded against their profile.

If they complete a shareable-link form and then book within 24 hours, and the submission is still unlinked, Petboost attaches that completed submission to the new booking instead of sending the form again.


SMS and Email Delivery

SMS Delivery

  • Contains a short message with the form name and a link

  • Link opens directly in the client's mobile browser

  • One-tap access - no login required

Email Delivery

  • Contains the form name, a brief description, and a link

  • Link opens in the client's browser

  • Serves as a backup if the SMS is not seen

Both channels are sent simultaneously. The client only needs to complete the form once regardless of which channel they use.

The form Responses tab has a Sent via column for every row. It shows SMS, email, both, Self-service booking when the form was collected in online checkout, or Not sent. If a send used a mobile or email, that destination is listed under the channel. If no channel is stored and the booking was not self-service, the column shows a dash.


Frequently Asked Questions

Q: Can I send a form without a booking?

A: Yes. Use Send Form from the pet owner's Fields & Forms tab, or share the form link directly.

Q: What if the client does not have a mobile number on file?

A: Only email delivery is sent. Add a mobile number to their profile to enable SMS delivery.

Q: Can I resend a form that the client has not completed?

A: Yes. Use Resend on the pending submission. That re-sends the same link. Do not use Send Form unless you intend to create a second pending.

Q: Does the client need a Petboost account to fill in forms?

A: Forms sent via link can be filled in without logging in. The submission is associated with the client's profile based on the form link.

Q: I unlinked a completed form from an appointment. Will it send again?

A: No. Detach / Remove only clears the appointment link. Status stays Completed. Once per client stays satisfied for that owner. Change frequency or Duplicate the form if you need another send.
